Sign Up For Advantage Program

Signing up for Carrollton Smiles Advantage Program is very simple and there is no fine print or exclusions!

  1. When you are at our practice, mention it to one of our team members and they can help you. Contact us for access to our patient portal and you can fill out our CSAP application form ahead of your visit
  2. After you sign up, your membership is active for 365 consecutive days. Your membership can be renewed after the expiration date.

  4. Your CSAP includes:
    • 1 airway scan.
    • 2 sets of cavity detecting X-rays.
    • 2 regular oral prophylaxis also known a regular dental cleanings.
    • 15% discount on all the dental procedures during the active period.

If you need a scaling and root planing, also called deep cleaning, you will receive a 15% discount on it. The 2 complimentary dental cleanings will be applied to the 4 month maintenance visits.

Please note: The Carrollton Smiles Advantage Program cannot be transferred or canceled.

General Information

  • Our Individual Adult VIP Membership Costs $350 per year.
  • Our Individual Children’s VIP Membership costs $235 per year

Your VIP Membership begins the same day that YOU sign up! It is active for 365 consecutive days from the activation day and makes you eligible for.

Our VIP Membership comes with several benefits that help our patients and their families get the treatments they need. Some of those perks include:

    • Unlimited annual maximums
    • No pre-existing condition exclusions
    • No waiting period
    • No pre-authorization required
    • 15% off all dental procedures!
  1. Our VIP Membership – Carrollton Smiles Advantage Program – can only be used at Carrollton Smiles!
  2. Additionally, Carrollton Smiles Advantage Program is NOT a dental insurance plan.
  3. Carrollton Smiles Advantage Program cannot be combined with other promotions.
  4. Payment for Carrollton Smiles Advantage Program is non-transferable and non-refundable.
